Collaborative Care (CoCM) is a specific type of integrated care to treat common mental health conditions in medical settings like primary care. Behavioral health conditions such as depression, anxiety, PTSD, alcohol or substance use disorders are among the most common and disabling health conditions worldwide.
As your attending physician group, we care about your physical and mental health. By enrolling in the program, your primary care provider, a social work case manager, and psychiatric consultants work together to make the best plan of care. There will be no face-to-face visit but rather the case manager will call you at least once a month to discuss your Plan of Care or the medication treatment being provided to you.
What is Behavioral Health Integration Services (BHI)?
is a new Medicare program that caters all Medicare beneficiaries. It allows medical professionals to provide a plan of care to patients with behavioral health issues such as depression, anxiety, and substance use disorder.
In this program, your primary care provider, a social work case manager, and psychiatric consultants work together to make the best plan of care. The case manager will call or visit you at least once a month to help you feel better and healthier.
